Chronic pain is a major societal problem and a common reason for healthcare visits and sick leave. Long-term opioid treatment is common but has weak evidence for non-cancer pain, meaning many patients may be treated inappropriately. Coordinated, multiprofessional approaches with roles such as care coordinators and clinical pharmacists are considered promising.

The study aims to optimize pain management through a person-centered team model in primary care. A controlled before-and-after study will be conducted in Region Örebro 

and Region Uppsala with 160 patients. The intervention lasts 24 weeks and is compared with usual care. The primary outcome is pain impact on daily life, with sick leave as a secondary outcome.

The study is expected to improve quality of life, physical activity, and work ability, and provide guidance for future care.
