Inflammatory bowel disease and translational gastroenterology

Our research aims to improve the quality of life of patients with inflammatory bowel disease (IBD, including microscopic colitis, and also of patients with other gastrointestinal diseases. Exploring the molecular profile of these diseases is an integral part of projects performed in our research group.

The overarching aim of our research is to identify markers for improved clinical decision making and to uncover pathophysiological mechanisms behind these markers or signatures of markers. Epidemiological and mechanistic links between various disease elements and across different phases of the diseases are examined. Our research programs are conducted in collaboration with national and international research groups, patient organisations, and private companies within the biomarker-, pharmaceutical- and digital sector.

The overarching goal is to increase knowledge, improve patient care, and enhance treatment of patients with liver- and biliary tract diseases, including autoimmune liver disease, liver cirrhosis of various etiologies, and liver cancer. Most ongoing studies in hepatology are conducted within the SweHep research network.

SweHep (formerly SILK) is an association of hepatologists from all Swedish university hospitals. Through collaboration in data collection and patient recruitment for various treatment studies, this network has enabled research on even rare liver diseases in Sweden. The organization was founded by Professor emeritus Gunnar Järnerot from Örebro. This research collaboration has resulted in several doctoral theses and more than 80 publications. At the gastroenterology section, Nils Nyhlin serves as Örebro’s representative in this hepatology research group, together with specialist physician and PhD candidate Ida Henriksson. Research nurses Anna Segerås and Anna Hjelte are also involved in the ongoing prospective studies.

We research how various interventions can improve metabolic health. Using different techniques, we study how important organs such as the brain, liver, and gut are affected by obesity and weight loss.

Research Goals We have facilities to study human metabolism, which allows us to conduct translational research aimed at understanding the development of obesity and metabolic diseases such as metabolic dysfunction-associated fatty liver disease (MAFLD). We examine physiological parameters, develop intervention programs, and implement them in a way that they can be maintained over time. Examples include prevention and treatment programs through the endoscopy unit, obesity unit, and online.

In several subprojects, we aim to address key questions such as:

  • What is the diagnostic value of gastroscopy, colonoscopy, capsule endoscopy, and double-balloon enteroscopy for different indications?
  • Can patients be better selected based on symptoms, psychometric data, or laboratory findings in order to reduce unnecessary procedures and prioritize those who should undergo endoscopy first?
  • What is the diagnostic value of other functional assessments such as anorectal manometry, esophageal manometry and pH-metry, liver elastography, and defecography for different indications?
    Clinical trials are also being conducted comparing different laxatives used in bowel preparation before colonoscopy.

Both Michiel van Nieuwenhoven and Nils Nyhlin are members of the recently established research network Colonoscopy Research Sweden (CRS). CRS brings together researchers in Sweden focusing on colorectal endoscopy, with the primary aim of conducting and supporting clinical research in colorectal endoscopy.
